EXTRACURRICULAR ACTIVITY PARTICIPANT INFORMATION
Each student participating in an extracurricular athletic activity must complete the UIL forms entitled “Pre participation Physical Evaluation – Medical History” and “Acknowledgement of Rules”. Each form must be signed by both the student and the student’s parent or guardian. Education Code 33.203(a) These forms are available from the coaching staff.
Each student participant and the student’s parent or guardian should be provided a copy of the text of Education Code 33.201-33.207 and the “UIL Parent Information Manual 2008-2009”.
